I understand that lash extensions, lash lifts, teeth whitening and spray tans are cosmetic services. Possible reactions include irritation, redness, swelling, sensitivity or allergic reaction, and results vary from person to person. A patch test is available on request before my appointment.

I agree to follow the aftercare instructions I receive. I understand Emma may decline or stop a service if she believes it is unsafe for me.

I release The Reset Ritual and Emma from liability for reactions arising from information I did not disclose or aftercare I did not follow. I understand the $15 deposit is non refundable.
